An advanced heart failure and transplant cardiologist cares for people whose heart failure no longer responds to conventional therapies and symptom management. Heart failure specialists perform procedures, manage complex treatments, including ventricular assist devices, and provide advanced heart care for people before and after heart transplant surgery. A heart failure cardiologist typically evaluates a patient’s medical and heart history, and explains heart failure and its management to patients and their families, performs a physical exam including assessing the heart, lungs, and blood vessels, orders and interprets laboratory, imaging and other tests including EKG (electrocardiogram, or ECG), echocardiogram, and cardiac catheterization, determines if a patient is a good candidate for a heart transplant or for devices that help the heart pump more effectively, prescribes medications and cardiac rehabilitation, performs advanced heart treatments, and manages patients before and after heart transplant, treats and monitors conditions that worsen heart disease and heart failure, such as high blood pressure, smoking, and high cholesterol, evaluates and manages potential heart donors, provides end-of–life care, and works closely with complex care teams to provide optimal care. Teams include transplant surgeons, transplant coordinators, pharmacists, social workers, psychiatrists, dietitians, specialized nurses, and cardiac rehabilitation specialists. Advanced heart failure and transplant cardiologists may also be known as cardiologists, heart failure specialists, heart failure cardiologists, and heart transplant cardiologists.
